Advise Beneficiary Form - ASIA ACH Credit/GIRO

This form is used to select the advice media and enter advise beneficiary contact information.

1. In the Advice to Name field, enter the name of the individual or organization you want to advise. 

2. In the Advice Media field, click the drop-down arrow and select one of the following delivery methods:  Mail, Fax, or Internet.

3. In the Fax Number field, enter the fax number if Advice Media is Fax.

4. In the Alternate Fax Number field, enter a fax number if Advice Media is Fax and an alternate fax number is available.

5. In the Contact Phone Number field, enter the contact phone number.

6. In the Destination (Zip Code) field, enter the zip code if Advice Media is Mail.  Note:  This field only applies to Thailand.

7. In the Internet Address field, enter the Internet address if Advice Media is Internet.

8. In the Mail to Name field, enter the name of the party to whom the payment is to be mailed.

9. In the Mail to Address field, enter the name of the party to whom the payment is to be mailed.  To view all information in this field, click the multiple line button.  Note:  This field is required if Advice Media is Mail.
